POST EXAM RELEASE FORM

As in past years, students will be allowed to leave school after they complete their exams IF they have a signed parental request form on file. IF you wish to have this privilege extended to your son(s)/daughter(s), please complete the following form and have it returned to school. Parents may cancel this release at any time during the school year. There is no requirement that you complete this form unless you so choose. Furthermore, you may decide not to make one or more of your son(s)/daughter(s) eligible for this privilege.

I RELEASE FORT LA BOSSE SCHOOL DIVISION, AND ITS STAFF AND AGENTS, FROM ALL LIABILITY FOR ANY INJURY, ACCIDENT, OR MISHAP WHICH MAY OCCUR DURING THE REQUESTED ABSENCE.
